Procedures of Tax Registration
I      Requirements of Tax Registration
1     Identification of investors. After the enterprise name is ratified, the investment promoter is required to fill in an “Identification Form” in duplicate which are to be submitted to the certification office for ID card verification (if the address on the ID card is not the same as the domicile, the current address shall be indicated) before being checked online at the police station, when the ID card copies of the enterprise’s legal person and shareholders shall be provided.
2     Notify the enterprise to receive the “Revenuer Qualification Certificate” training after its name ratified. The “revenuer” is required to take a copy of ID card and a photo when attending the training.
II      Materials Needed for Filling and Production of Tax Registration Certificate (Two copies shall be provided for the following materials)
(1) Business License of Enterprise legal person;
(2) Organization code certificate;
(3) ID cards of the legal person, shareholders and revenuers (Note: If the addresses on the ID cards of legal person and shareholders are not the same as current ones, the current telephone, address and postal code shall be written under the copies, but that of the revenuer shall be the same as the fillings when attending the training);
(4) Household Registry of Legal Person;
(5) Original of housing and site rental contract and rental invoice (One is the contract of place of registration, the other is the contract of actual place of operation);
(6) Corporate rules;
(7) Capital verification report;
(8) The enterprises invested by legal person are required to provide the Tax Registration Certificate and the Domicile Management Certification of the investor.
III     Materials Needed for Report of Tax Registration
 (1) Tax Registration AB Form (3 copies);
(2) “Revenuer Card” (1 copy), or shared card attached with application (1 copy);
(3) a     Tax Items Acceptance Form (2 copies) b Return Receipt of Tax Items Application Materials (2 copies)；
(4) Withholding Individual Income Tax and Payment of the Withheld Tax Application Form (Jiading District) (3 copies);
(5) Verification Form for Small-sized Commercial Enterprise Value-added Tax Application (2 copies);
(6) Letter of Business Authorization (1 copy);
(7) ID Card Certificate (2 copies);
(8) Domicile management materials (1 copy);
(9)        Domicile Management Verification Form (2 copies) (Note: Those enterprises of which the investment shareholders are natural persons, as well as privately-invested enterprises and joint ventures, are excluded)；
(10)      Survey Form of Place of Registration and Operation;
(11)      Tax Opening Registration and Audit Form;
(12)      Tax Registration Negotiation Notes;
(13)      Photos of the place;
(14)      Tax Categories Check Form
IV    Materials Needed for Administrative License
 (1) Tax Administrative License Application (2 copies);
(2) Tax Administrative License Return Receipt (2 copies);
(3) Tax Administrative License Acceptance Notice (2 copies)
(4) Tax Administrative License Acceptance Notice Return Receipt (2 copies);
(5) Tax Administrative License Decision (2 copies);
(6) Tax Administrative License Decision Return Receipt (2 copies);
(7) ID card copies of legal person or accountant and revenuer (2 copies);
(8) Moulage of invoice seal (2 copies).
V    Materials Needed for Branch
 (1) Branch Business License;
(2) Branch Code Certificate;
(3) ID card and household registry of the Principal;
(4) Registration leasing agreement or property right certificate.
Note: The related materials of the headquarter must be provided when the branch is in transaction, unless it is transaction at the same time as the headquarters.
